Development and evaluation of an intervention for the structured involvement of general practitioners in aftercare after medical rehabilitation - rehabilitation follow-up consultation ReNaGe

Background

In order to ensure the long-term effects of medical rehabilitation, good follow-up care is essential, which is why aftercare services are provided by the statutory pension insurance and other service providers in accordance with social legislation. Many studies show considerable problems in the use of aftercare services. The rehabilitation patient's family doctor can play an important supporting role here by motivating them to make use of suitable aftercare services and by supporting their use. 

Aims and goals

The project will therefore design and evaluate a structured short intervention in the form of a guideline-based follow-up consultation with the family doctor. The objective is to investigate how and with what result this short intervention can be integrated into primary care after medical rehabilitation. 

Methods

A mixed-methods research approach is planned. In the first phase, the intervention concept is developed with the participation of general practitioners and formatively evaluated. This is followed by a summative evaluation with a cluster-randomized crossover design. Randomization is carried out at the level of the cooperation facilities, with the intervention or control condition changing after a defined period of time (9 months) in the facilities. Rehabilitation patients of the German Pension Insurance North Bavaria in the intervention condition receive an invitation to a follow-up consultation with their family doctor 4-8 weeks after discharge from rehabilitation. The primary outcome is the use and implementation of aftercare recommendations. Other variables collected include self-reported work ability (secondary outcome), self-efficacy, functional status and return to work status. Data are collected at the end of rehabilitation, after the follow-up consultation and 3 months after discharge. We also investigate whether the level of information of general practitioners on medical rehabilitation and follow-up care can be increased through the follow-up consultations.
